Transcript Request Form

Use this form to request a copy of your transcripts to be sent to a college or place of work. The forms can be turned into the Counseling Office at WHS or emailed directly to your student's counselor. If you are a graduate of WHS the cost is $1.  If you are a current student there is no cost.

Letter of Recommendation Form

Use this form when you are requesting a letter of recommendation. Fill it out and give it to the person you are asking to write a letter of recommendation for you.  Make sure to include any deadlines so they know their timeline.
